Homes not hospitals.
Everyone has the right to their own home
Autistic people and those with learning disabilities have the right to live independently in their own homes, surrounded by community and choice. At Active Prospects, we transform lives by supporting people to move from hospital settings into their own homes – with a proven 99% success rate.
Ready to make community-based care a reality for someone you care about?
The problem
Too many autistic people and those with learning disabilities remain trapped in hospital settings when they could be thriving in their own homes. Long hospital stays are isolating, preventing people from building meaningful relationships, developing new skills, and living full, independent lives. The current system fails because the right housing and community support aren’t available.
Our Solution
Active Prospects specialises in person-centered support that helps people transition from hospital to independent living in their own communities. With over 30 services across Surrey and West Sussex, we provide tailored support that enables people to gain confidence, develop independence, and build meaningful relationships. We don’t just provide housing – we create pathways to genuine independence and belonging.
Our services
We provide housing support to around 300 people each year with learning disabilities, including those with physical disabilities, sensory impairments, mental health needs, behaviours of concern, or who are autistic. This includes residential care homes, supported living flats, shared houses, bungalows, and people’s own homes. We own many of the properties and lease others from high-quality housing providers.
