Active Prospects release their Wellbeing Report

We have recently released our Wellbeing Report which summarises our approaches and our key outcomes for people who need intensive support, formerly known as transforming care.

The report outlines our approaches and our key outcomes while highlighting some of the incredible achievements, such as a 64% reduction in significant behaviours that concern over 12 months. But, the report also has insightful real examples of people we support who are living in their own homes and flourishing under our care, after coming through such difficult times within their lives.
