Our approach to personalised care
We are a regional leader in enabling people, often with complex needs and behaviours of concern, to leave hospitals to live in bespoke homes in their local areas. We have supported 48 people in doing this in the last three years. We deliver a 68% reduction in people’s significant behaviours of concern through our support programmes with 94% of placements for people leaving hospital, still in their homes after two years. Every person has a set of needs, and our approach is to tailor our services and care plans to suit each person we support. By adopting flexible and responsive care strategies, we can provide support that adapts to the evolving needs of those in our care. For example, Just Roaming allows us to reduce intrusive checks by monitoring activity in a way that respects privacy and autonomy. This innovation is just one example of how we strive to balance support and independence.
Amy’s experience
Amy’s story shows how our innovative approaches directly benefit the people we support. Amy, who has multiple complex conditions including autism and ADHD, faced significant challenges with traditional care models. By integrating tailored monitoring systems into her care plan, we were able to provide Amy with the space and privacy she needed, while still ensuring her safety. This approach led to a significant reduction in self-injurious incidents and allowed Amy to gradually build her independence.
